1) You are hereby notified that a Petition for Declaratory Judgment has been commenced against you in the Circuit Count of Miller County, Missouri, on behalf of Plaintiff Crum & Forster Specialty Insurance Company, seeking a declaration from the Court that an insurance policy it issued to Lucy’s, LLC does not provide coverage for the dram shop claims against Lucy’s in the matter David Whitney, Jr. v. Benjamin Jans, et al., Miller County Case No. 21ML-CC00043-01.
2) The parties to this action, Cause Number: 25ML-CC00030, filed in the Miller County Circuit Court are: Plaintiff Crum & Forster Specialty Insurance Company, and
Defendants David Whitney, Jr., Benajmin Jans, Lake Management Group, Inc., Foresight Acquisitions, LLC, Advanced Precision Consulting, LLC, and Lucy’s LLC.
