REQUEST FOR PROPOSAL
The Miller County Ambulance Districts is currently seeking proposal for the following:
Garage alteration at the St. Elizabeth ambulance base:
Scope of work includes, but is not limited to:
• Remove garage door
(to be reused)
• Remove ceiling
• Remove roofing materials and roof deck
• Remove roof trusses (to be salvaged and reinstalled)
• Raising 4 walls 36”
• Install siding on roof extension area (match siding as close as possible)
•Reinstall roof trusses
• Re-deck roof
• Install new 30 lb. felt
• Install drip edge
• Install new 30 year Architectural shingles
• Install new roof ridge vent
• Install insulation
• Install drywall as necessary
• Install garage door & garage door opener
Contractor shall be responsible to obtain necessary building permits.
Contractor shall be responsible for all debris clean up and disposal.
Closing date for proposal submission will be June 15, 2023 at 4:00 p.m.
Proposals will be reviewed by the Miller County Ambulance District Board of Directors and Administrative team. Upon selection, contractor will be notified immediately with work to commence as immediately.
